SOULARD, Mo. (KMOV) - Three years later, the Chemical Safety Board (CSB) released its final report after a massive boiler exploded at the Loy-Lange Box Company on Russell Boulevard, leaving four dead in Soulard.
On April 3, 2017, a large industrial boiler exploded at the Loy-Lange Box Company on Russell Boulevard. A substantial piece of the boiler then traveled around 500 feet and crashed through the roof of the Faultless Healthcare Linen building.
Kenneth Trentham, 59, was killed in the initial explosion at the box company. When the equipment crashed into the Faultless Healthcare Linen building it struck two employees, 43-year-old Tonya Gonzalez-Suarez and 46-year-old Christopher Watkins, killing them. A fourth person, Clifford Lee, 53, was trapped under the hot boiler after it crashed into Faultless Healthcare Linen. He later died as a result of his injuries.
The CSB discovered an area of the failed pressure vessel had thinned due to a known corrosion mechanism that was poorly controlled and Loy-Lange repeatedly ignored clear warnings that corrosion was causing major problems within its operations.

CHICAGO (CBS) -- The Department of Chemistry and Biochemistry at Loyola University was evacuated due to a hazardous materials situation.
The campus building at 1068 W. Sheridan Rd. was evacuated Monday afternoon.
Loyola University Chicago said everyone got out safely. Emergency crews secured the scene by 5:15 p.m. after clearing out any dangerous fumes.
There were no injuries or transports, according to the Fire Department.

Aug. 8‰??Employees at a business at 33 South Hyde Park Boulevard are monitoring a tanker containing sodium following a Hazmat incident Monday morning.
Falls firefighters were called to the business shortly after 8 a.m. Monday after employees there reported that a cap on a tanker containing sodium malfunctioned, allowing moisture into tanker. Th reaction between the water and the sodium blew the cap off and erupted in flames.
The fire was out when firefighters arrived but out of concerns about the spread of water within the tanker, units from the Niagara Falls Air Reserve Station and Niagara County Hazmat. The business and another business across the street were evacuated.
A chemical foam was sprayed into the tanker and it was covered to guard against any rain falling Monday.

Three people have been hospitalised after a chemical spill at a nuclear facility in southern Sydney.
About 250 millilitres of sodium hydroxide was spilt from a pipe at the Australian Nuclear Science and Technology Organisation at Lucas Heights on Friday morning.
An ANSTO spokesman said three staff members had been taken to hospital for observation.
The incident occurred in a nuclear medicine manufacturing building that is not associated with the OPAL nuclear reactor, the spokesman said.

ABINGTON TOWNSHIP, PA ‰?? A county hazardous materials team had to assist a local fire department in this eastern Montgomery County community this past weekend after an emergency call came through for an overheating incident.
The Montgomery County Hazardous Materials Response Team came out Saturday morning to Abington Township to assist the Edge Hill Fire Company on a call at a local private facility that handles and supplies research materials, according to the Montgomery County Department of Public Safety.
The business was not identified by officials.
The DPS said that materials at the facility had overheated while they were in storage.
The nature of the types of materials that were affected was not released by the county.

TRACES OF 2010 DEEPWATER HORIZON OIL SPILL ARE STILL DETECTABLE IN 2020

Small amounts of highly weathered oil residues from the 2010 Deepwater Horizon disaster were still present in the surroundings ten years later, shows a new report. Crude oil is a complex mixture with many components that undergo chemical reactions in the environment. These transformed chemicals, as well as longer persisting oil products, can impact local ecosystems and a better understanding of the fates of these molecules can help future cleanup efforts.
The oil spilled during the Deepwater Horizon disaster in 2010 was largely transformed by the end of that summer, reports a new study in Frontiers in Marine Science. But some small quantities of chemical residues still persisted in the environment even ten years later. This latest study follows the varied fates of the leaked petroleum components, providing important insights for future spills and cleanup efforts.
"The better we understand the chemicals and their chemical reactive properties as well as their physical properties, the better we will be able to mitigate oil spills and understand and detect environmental damages from oil spills," said first author Prof Edward Overton of Louisiana State University.

The Wichita Falls Fire Department, Sheppard Air Force Base Fire Department, Wichita County Sheriff's Office deputies and several ambulances responded Sunday afternoon to a hazardous materials situation at the Vitro Architectural Glass plant on Central Freeway. The incident was updated to a structure fire, according to the WFFD Facebook page.
According to a statement released from Vitro, a leak from a furnace allowed molten glass to escape and ignite material under the furnace. Six plant employees were taken to the hospital for treatment.
The release noted the incident did not involve the leakage of natural gas, but molten glass used in the glass making process.
