Submitting incidents to a lessons learned site, even an internal one,
can be problematic for some institutions. During my postdoc there was a
fire in one of our labs. Turns out that scaling up an LAH reduction just
before group meeting isn't the best idea. The building manager was
furious when he found out I took photos. Few people in the department
ever found out about the incident.
