DOT requires TAKING a test. The regulations actually say nothing about PASSING, although one might argue that that is implied in the taking.

 

When I did remote DTOT HazMat training I had a manager present with the employee to confirm presences with a sign-in sheet and forward the sign-in and the tests to me for scoring

Similar to Monique, I've made a "self certification" form in the past listing the title of the seminar and date attended.  I've based this on our State regulators that do on-line webinars and encourage downloading and using their self certification form. 

 

However, that might not work for everything, especially DOT since it is my understanding that DOT training requires documented passing an examination/ quiz.
