Wayne: some schools or companies add where specifically the course was held and at what time, especially if there are multiple sessions on the same day; a numerical identifier for the student and/or employee, e.g, student ID, union ID or only last four digits of SSN if using that, and a comments section to include something specific to the presentation such as having a person using sign language, etc. Also, if the course if required by either a state or federal law include that along with the course title.
