NORTH BEND, Wash. -- The Washington State Patrol is investigating a serious mishap that occurred Oct. 25th at its fire training academy in North Bend.
During a training exercise, firefighters mistakenly pumped jet fuel instead of water onto a flaming mock-up of an airplane crash.
KING 5 obtained video that shows an enormous fireball erupted when the fuel hit the flames at the training site.
?When the firefighters put water on a fire that had been deliberately developed for training the fire got bigger instead of smaller,? said Bob Calkins of the State Patrol, which is investigating the incident.
Calkins said investigators believe that the academy?s oil/water separator ? which recycles the water used for fire training exercises ? did not correctly filter the jet fuel used for the exercise from the reclaimed water.
So crews filled their tanks with water tainted with jet fuel -? a combustible combination.

The Saratoga Springs Planning Board heard for the first time recently plans for a proposed facility in the W.J. Grande Industrial Park that would dilute hydrogen peroxide for use at GlobalFoundries. The industrial park is off Geyser Road near the Saratoga Performing Arts Center.
Highly concentrated hydrogen peroxide would be transported from Texas to the industrial park by rail, to be diluted with water and sold to GlobalFoundries.
The hydrogen peroxide would arrive in Saratoga Springs 70 percent concentrated and leave at 31 percent. The hydrogen peroxide found in pharmacies is 3 percent concentrate.
Currently, GlobalFoundries receives already diluted hydrogen peroxide in truck shipments from Texas.

Manufacturers have been told to stop using a chemical found in everyday cosmetics and cleaning products after it was blamed for an ?epidemic? of painful skin allergies.
The preservative ? known as MI ? was used in a wide range of moisturisers as well as make-up and baby wipes.
Dermatologists linked it to a steep rise in allergies known as contact dermatitis, where the skin becomes red and itchy and can sting and blister.
They estimated that one in ten patients they were seeing with eczema or dermatitis was allergic to the preservative and called for it to be banned.

TWO chemical companies ended up with a bill of £120,000 after a court heard how workers suffered life-changing conditions by being exposed to a poisonous gas.
The gas was used in the manufacture of a chemical for an globally-marketed angina medication, which also escaped into the atmosphere.
Pharmaceutical manufacturers Archimica Chemicals Ltd, of Prince of Wales Avenue, Sandycroft, admitted six charges brought by the Health and Safety Executive and Euticals Ltd, which later took over the site, admitted two charges brought by Natural Resources Wales of failing to comply with notices, allowing chemicals to escape from the site and failing to notify the authorities when it occurred.
Both are in liquidation. The liquidator had entered guilty pleas to all charges on an earlier occasion at Flintshire Magistrates Court at Mold.

ACID was spilt from a container in a church car park in Faringdon this morning.
Fire crews were called to the United Church off Gravel Walk at around 10am by members of the public who had seen the chemical spill.
Two fire engines attended from Faringdon and Oxford along with a specialist officer.
They were able to contain and clear up the 25 litres of hydrochloric acid from the car park wearing protective equipment.

A chemical clean-up is underway at the site of a fertiliser factory fire in Auckland.
The Te Atatu Peninsula factory was gutted by fire.
RADIO NEW ZEALAND
More than 60 firefighters were sent to the blaze which gutted the plant off Wharf Road on the Te Atatu Peninsula on Sunday afternoon.
Some of the firefighters had to be decontaminated after getting close to melted chemicals used to make fertiliser.
The Fire Service has largely contained the chemicals, some have seeped into the stormwater system, and contaminated water may have reached Totara Creek.
The creek is now being monitored, while the council's pollution response team is at the burnt-out factory.

ALSIP, Illinois ? An official with the suburban Chicago chemical plant where an explosion triggered a fire says that emergency workers have extinguished the blaze.
Blue Island Phenol plant manager Bill Moffatt says firefighters put the fire out by 2:20 p.m. Friday, nearly three hours after arriving at the plant in Alsip.
Moffatt says a leak of "gas material" triggered the explosion and fire in an area where perhaps 24 people were working, injuring two of them.
He says they were taken to an area hospital where they are in stable condition. He says he did not have any other details.
A man who lives about three blocks away tells the Chicago Sun-Times he heard a muffled noise and his entire house shook. He says he thought it was an earthquake.

MERCED COUNTY ? A ranch worker was apparently grinding a 300-pound gasoline tank late Wednesday afternoon when the tank exploded, killing the worker and seriously injuring two other workers, a state health and safety official said Thursday.
The names of the man who was killed and the two who were injured have not been released.
Peter Melton, a spokesman for the state Division of Occupational Safety and Health, was careful to note that the investigation is still in its early stages and said details may change as the inquiry progresses.
One of the workers suffered severe burn injuries and another suffered ?less serious injuries,? Melton said.

Federal health and safety inspectors have opened a second investigation in less than a year into working conditions at a local industrial and hazardous waste management plant following an explosion there Wednesday night ? the second in 12 months.
Last year, a Dec. 28 explosion rocked Environmental Enterprises Inc., fatally wounding 20-year-old Zachary Henzerling of Colerain Township and injuring another employee.
The blast this week was smaller in scale; no one was hurt and the ensuing fire was kept in check by a sprinkler system until firefighters arrived at the site in Spring Grove Village.
But inspectors with the Occupational Safety and Health Administration will again investigate even as Environmental Enterprises continues to fight OSHA?s findings in last year?s explosion. In that case, OSHA cited the company with 22 health and safety violations, including four ?willful? violations ? finding the employer either ?knowingly failed to comply with a legal requirement? or ?acted with plain indifference to employee safety? ? and 18 ?serious? violations, hazards that ?could cause an accident or illness that would most likely result in death or serious physical harm,? according to OSHA standards.

LAS VEGAS (Las Vegas Sun) -- A worker was seriously burned this morning while spray painting the clubhouse of a homeowner?s association in the western Las Vegas Valley.
Las Vegas Fire & Rescue spokesman Tim Szymanski said the fire erupted at 10:17 a.m. at the Queensridge Homeowners Association building in the 800 block of Palace Court.
A group of workers were painting the inside of the building with a commercial paint sprayer and had placed thin plastic sheets around the area to prevent overspray.
The accumulated paint fumes were apparently ignited by the pilot light inside a natural gas fireplace in the building.
A fire sprinkler doused the flames before firefighters arrived.

ODON ? A natural gas explosion at Green City Cabinets just outside of Odon Saturday sent one man to the hospital to be treated for burns. Owners of the family owned and operated business said that a natural gas leak inside the engine building caused the building to explode. The man injured, Levi K. Graber, sustained burns that are now being treated at home with home remedies. The engine building has already been cleared from the property.

The U.S. shale gas boom is reverberating across Britain?s chemical industry, the nation?s second-largest export earner.
The 20-billion pound ($32 billion) chemicals business is losing sales to lower-cost competitors such as in the U.S., where new supplies from domestic shale drilling have reduced prices for natural gas, the fuel used in making chemicals such as plastics. By 2020 the chemicals industry in the U.S. will be 21 percent larger than in Europe, from near parity now, according to the American Chemistry Council.
The price of gas, also used to make electricity, in the past month averaged about two-thirds less in the U.S. than in Britain, the steepest discount in five years. That?s giving Americans an edge over U.K. chemicals makers such as Ineos AG, the largest. BASF SE (BAS), India?s Tata Chemicals Ltd. and Lotte Chemical Corp. (011170) of South Korea have closed plants in Britain this year.

CINCINNATI -- An explosion and fire at a local business in Winton Place Wednesday night required the assistance of HAZMAT.
District Fire Chief Rob McWilliams said the blast happened shortly after 11 p.m. at Environmental Enterprises Inc., located at 4650 Spring Grove Avenue.
Employees told fire crews arriving at the scene an explosion and fire occurred in a flammable liquid storage area inside the building.
Investigators say the sprinkler system went off and contained the fire to one barrel.
Crews quickly extinguished the fire and isolated the burning barrel.
All employees were evacuated safely and no injuries were reported.

irefighters from across the San Gabriel Valley and Los Angeles County responded to a HazMat situation early on December 9, 2013, which required the evacuation of nearly 200 employees from nearby industrial companies.
Monrovia fire officials told the San Gabriel Valley Tribune that a three unknown chemicals had been mixed together to create a cocktail of hazardous gas.
The chemicals have since been identified as cyclo aliphathic resin, boron trifluoride, and ancamine.
At least 50 of the evacuated employees complained of headaches and nausea from the fumes. Some were taken to hospital.
The fumes, carried through a ventilation system, originated at Air Logistics Corp, which mixes chemicals as part of its manufacturing processes in Southern California.
