I''d imagine you would want two kinds of checklists/procedures - one for weekly use within the lab itself, and a more comprehensive one that a safety committee or EHS department would use.

I haven't run a lab in over ten years now, but I dusted off the one that I used to use on a weekly basis within my research group and posted it here:    http://www.il pi.com/safety/inspection_checklist.pdf

After performing an inspection for each lab, I would staple a "READ AND CHECK OFF" Post-It size note to the reports and ensure that each group member reviewed it.  This was the same protocol I used for other group memos - important research papers, university notices etc. etc.

The idea was to keep it short and sweet - something that could be ticked off in a few minutes and help stay on top of situations before they could become serious.  There are probably minor tweaks I would make to the content given discussions on this list over the past decade but the idea still works.
