The student most seriously injured was igniting the experiment.  Insurance company and the state are apparently investigating, so we should eventually learn more.  But https://www.tribstar.com/news/local_news/chem-experiment-sends-4-students-to-hospital/article_e9af8407-6708-5b87-9465-dacd9d13ff8a.html has the following quotes:


"The goal was to change the color of the flame, but the reaction was different. They used a different material without changing mixing ratios, Hooper said, based on his son's account."

"The chemicals used the day prior [Monday] was in fact used in an experiment done many times over the years. However, the next day, one of the chemicals was replaced by another for a first time experiment. My son just happened to be the one completing the experiment by lighting it,"

Several reports mention they were looking at the labeling of the chemicals as well.
