In today's episode, we discuss another in our series of foundational papers: 'Applying Systems Thinking to Analyze and Learn from Events' published in a 2011 volume of Safety Science by Nancy Leveson. Leveson is a renowned Professor of Aeronautics and Astronautics and also a Professor of Engineering Systems at MIT. She is an elected member of the National Academy of Engineering (NAE). Professor Leveson conducts research on the topics of system safety, software safety, software and system engineering, and human-computer interaction.
We will review each section of Leveson's paper and discuss how she sets each section up by stating a general assumption and then proceeds to break that assumption down.We will discuss her analysis of:
Safety vs. Reliability
Retrospective vs. Prospective Analysis
Three Levels of Accident Causes:
Proximal event chain
Conditions that allowed the event
Systemic factors that contributed to both the conditions and the event

HYDROGEN SAFETY FUNDAMENTALS

Tags: industrial, discovery, environmental, hydrogen
As the number of producers and end-users of hydrogen grows, so too does the importance of its safe handling and use.
Hydrogen use has dramatically increased in recent years, especially as it has grown in popularity as a transportation fuel. Although hydrogen has been used in industry for decades, its use as a fuel for vehicles or for stationary power generation in consumer environments is relatively new (1). Hydrogen is traditionally used to make ammonia for fertilizer, remove sulfur in fuels in oil-refining processes, manufacture silicon chips, and hydrogenate oils in the food industry. It is also a common reactant and byproduct in the specialty chemical and pharmaceutical industries.
Many chemicals have inherent risks that must be managed, and hydrogen is no exception. If managed appropriately, hydrogen can present less risk than other fuel sources. For example, hydrogen is nontoxic. And, because hydrogen is lighter than air, it dissipates rapidly when it is released, unlike hydrocarbon-based fuels. However, although it is a commonly used and understood molecule, hydrogen presents unique safety challenges. Therefore, it is important for every engineer and facility support specialist to know and understand how to identify and mitigate hazards presented by hydrogen.

TAYLORSVILLE, Miss. (WDAM) - A fire that started inside a mixing tank Friday morning at Georgia Pacific Chemicals in Taylorsville was quickly brought under control.
No was injured and emergency responders did not have to evacuate anyone from the grounds or surrounding area,Taylorsville Police Department Chief Gabe Horn said.
Georgia-Pacific spokesman Rick Kimble said a small fire had started inside a 'reactor,' a tank about the size of a mid-sized vehicle where chemicals are mixed for use in various resins and other woodworking products.
Kimble said the fire burned through a seal to reach the exterior of the tank.
That's when units from Taylorsville, Magee, Mize and other nearby volunteer departments arrived on the scene.
Horn said the uinits used foam to extinguish the fire, which he said is standard operating procedure
'I want to make this very, very clear that there was no explosion at this site,' Kimble said. 'The reactors do have pressure relief valves, and when they go, which this one did like it was supposed to, it makes a very loud noise.
'That could have been what some people mistook for an explosion.'
