You're correct - in my mind that is a key challenge in writing Lessons Learned reports. Is the goal of these report to identify a "root cause" or corrective action related to a specific incident, or is it to raise awareness of the safety elements of the situation being discussed that contributed to the event?
I think that the DOE report is more helpful for the second purpose than the first.
