FUMIGATION GASES LEAK, AREA EVACUATED

Five nearby residents were treated after feeling ill from toxic fumes and others were evacuated after a flap opened to release gases from a house being fumigated Tuesday night in the 400 block of South Kihei Road, police said.

Kihei patrol officers responded to the 7:10 p.m. call.

After the Maui Fire Department and its Hazmat team determined the area was unsafe at 7:28 p.m., officers shut down South Kihei Road from Kaonoulu Street to Ohukai Road and began evacuating nearby residents.

Fifty to sixty people were evacuated, police said. The five people who felt ill were treated in a safe area by medics, police said. One of them was transported in good condition to Maui Memorial Medical Center.

RFP responded to a home on Glide Street at around 8 p.m. Thursday after a resident said they mixed household chemicals in an attempt to unclog a sink drain.

Concord firefighters responded to 7 Eagle Square for a reported chemical spill Thursday morning.

The incident at the New Hampshire Office of Licensing and Board Administration required an evacuation of the building but did not result in any injuries, Concord Fire Chief Guy Newbery said. The state office is located on the third floor of the old city stable building.

Newbery said the first firefighters on scene learned that a box holding bottles of assorted chemicals ‰?? including products containing mercury and arsenic ‰?? had been damaged during transport. As a result of the damage, some chemicals mixed.

‰??Fire Department crews worked to identify the exact products involved and determine if there were re-activity issues,‰?? Newbery said in a statement. ‰??The chemicals were identified and re-packaged for proper disposal.‰??

REDDING, Calif. ‰?? The Redding Fire Department is taking steps to prevent the spread of the novel coronavirus (COVID-19) by using UV lights to sanitize their trucks and equipment.

It's a move that allows the fire squad to ditch traditional chemical cleaners and sanitize medical equipment, computers, and any other hard surfaces in as little as 10 seconds.

In operation for just two weeks, the light has the potential to become a standard for sanitation in the department.

"I think it will always be an option from here on out just because we made the purchase and it would be irresponsible for us not to continue to use it," said Fire Captain Rik Valles. "But right now, the thing that's in our face is the COVID and this is how we're responding to it."

FEDERAL WAY, Wash. - Crews with South King Fire were dispatched to Wild Waves Thursday afternoon for a possible chlorine spill.

According to the fire department, the liquid spilled was determined to actually be sodium hypochlorite and not chlorine.

Crews say a total of 2,500 gallons of the liquid were spilled.

There are no reported injuries.

AGAWAM, Mass. (WWLP) ‰?? Multiple agencies were called to a hazmat incident at a residence on Tom Street in the Feeding Hills section of Agawam Thursday night.

The Agawam Fire Department said firefighters, police, and the Massachusetts Department of Fire Services Hazardous Materials Response Team are at a residence on Tom Street working on the incident.

DFS spokeswoman Jennifer Mieth confirmed with 22News the hazmat situation was a pool chemical accident. She added a lot of people are new pool owners this year and that details about safe pool chemical storage can be found here.

Agawam fire chief Alan Sirois told 22News two people were taken to the hospital with unknown injuries.

BRADENTON, Fla. (WWSB) Multiple agencies were called to the Genesis II Church of Health and Healing in Bradenton, Fla. in connection with search warrants.

Back in April, the FDA issued an injunction against the church for distributing a ‰??Miracle Mineral Solution‰?? that officials say was intended to treat COVID-19. The solution contains chlorine dioxide content equivalent to industrial bleach. Four individuals were named in the injunction, Mark Grenon, Joseph Grenon, Jordan Grenon and Jonathan Grenon.

According to Chief Brian Gorski of the South Manatee Fire Department, Hazmat crews were called to assist with warrants alongside local law enforcement and federal agencies. The warrants were served at the church location at 2014 Garden Parkway.

Chief Gorski said that Hazmat was there to help with the identification of chemicals found on site.

The crews found:

50 gallons muriatic acid

22 gallons of the finished ‰??Miracle Mineral Solution‰??

8,300 pounds sodium chlorite

CAMBRIDGE ‰?? Investigation into the cause of a fire has been on hold while hazmat cleanup personnel work to secure the Egide USA building on Washington Street in Cambridge where a fire began Wednesday evening, July 1.

Rescue Fire Company Chief Adam Pritchett said a private clean up company was still pumping out water on Monday evening, July 6, and as of Wednesday, July 8, was still securing the building from any hazmat threats so that an investigation can proceed.

6 INJURED IN HUGE BLAST AT CHINA FIREWORKS FACTORY

Powerful explosions rocked a fireworks factory in southwest China on Wednesday night, leaving at least six people injured, authorities said.

A billowing yellow cloud rose over the factory in the city of Guanghan in Sichuan province after the first blast and then thick, black smoke poured from the plant, according to images broadcast by state channel CCTV.

This was followed by a second explosion.

Six people were injured, including two with serious wounds, the Guanghan city government said in a statement on social media.

The fire was brought under control and the cause of the incident, which occurred at 9:10 pm, is under investigation, the government said.

Here, we wish to report a serious explosion during large-scale preparation of an amine by alane (AlH3) reduction of a nitrile bearing a CF3 group. The accident took place during the reduction of a 1 mol batch of 1-methyl-3-(trifluoromethyl)-1H-pyrazole-5-carbonitrile. At the stage of decomposition of the reaction mixture after reduction with MeOH, the reactor was destroyed by detonation; one researcher was seriously harmed by shatters and burnt by flames. This Case Study describes this incident in detail and discusses how we can prevent similar incidents.

Its purpose is simply to raise awareness of major incidents in the process industries, to promote the importance of root cause analysis and to catalyse cross-sector sharing of lessons learned and good practices. The following definitions were used in development of these 1-Pagers:

Near Miss is an undesirable event which, under different circumstances, could have resulted in a process safety incident.
Immediate Cause (or Basic Cause) is a sub-standard act or condition that led directly to the incident.
Critical Factor is an undesirable act or condition which, if eliminated, would have prevented occurrence or reduced the severity of the incident.
Root Cause (or System Cause) is an organisational failing that created circumstances or conditions enabling the incident to take place.
