Thanks to everyone for sharing their institutional unattended operations forms with me. I have collected 12, including those I found via googling. In addition to pre-formatted forms, there are also a variety of schools that have policies for unattended operations that outline a set of expectations without providing a uniform format for that purpose. Prudent Practices also includes a small section on the topic on Page 62 that could be the basis for a "continuity of operations" checklist.
In general, the topics addressed on the forms are reasonably consistent, vary in terms of the amount of detail required to complete the form.
The two questions raised during the list discussion:

> >I am having trouble with them putting them on the equipment or hood and never removing them. Now, they have become just another sign that is ignored. Anyone else have this problem?
did not seem to be addressed in a systematic way in the resources I've reviewed.
It seems to be that there is opportunity for improving the "state of the art" in this regard.
