I've seen my unattended experiment form posted and never removed, although since wrote the form so that it's really for a specific operation, there's not too much of that problem. It would probably be a good idea to put an expiration date on it-I'll have to add that to my form.


You can get the model I give out to labs here: https://labsafety.jhu.edu/2016/07/29/unattended-experiments/. It was originally written for teaching labs (after a minor incident with an unattended experiment), but it's adaptable to research as well. This is not required by policy, but provided as a resource for the labs.

Our policy is to include these on any reactions that are going and any item that is left on that heats (heatblocks, autoclaves, furnaces, etc) when the person responsible is not in the room (for lunch, overnight-).  However, I am having trouble with them putting them on the equipment or hood and never removing them.  Now, they have become just another sign that is ignored.  Anyone else have this problem?

I know that several campuses and/or chemistry departments have developed placard systems for chemical operations which are left unattended. The purpose of these signs is in case an incident occurs when the person who set up the process is not immediately available to answer questions about it.
 
I am interested in developing a list of these efforts in order to understand what level of detail people have determined is best suited for their needs in this respect. If you have developed a formal system that serves this purpose, I would appreciate it if you would share an URL to the form or a PDF copy of the form. I'll provide a list of what I receive to the list later this week.
