Dear Colleagues:
All of us have stories to tell of things that went wrong and of things that went right. The Journal of Chemical Health & Safety is seeking those stories as case studies in safety.
Back in the day when I was a nuclear engineer with the US Navy we had a fleet-wide incident reporting system. The purpose was to identify the challenges that we encountered that may have an impact on safety. Incident reports were required reading for all engineers and engineering technicians.
Identifying lessons Learned are a vital part of the safety community. There is no repository of lessons learned for lessons learned pertaining to chemical safety, though we have all have lots of stories that should be told so that our community can more easily learn from each other. The more we know about processes, the safer we can make those processes.
I recognize that there can be a litigious nature to incident reporting here in the US. The Journal will work with authors to anonymize and/or sanitize information in articles that could be problematic for authors or organizations.
