If your institution is in the Northeast--Radiation Safety Associates in Hebron CT is where I received my 40 hour course and I thought they did an excellent job with hands on activities as well as 'classroom' talks--this was in 2001, so it's been a while. I have called/emailed them with questions ever since and they've been great.
