We had good luck with Chemdura labels coupled with a chem resistant resin printer ribbon. These might satisfy your first two requirements, but #3 might be a little more challenging.

Does anyone have any specific recommendations for bar code label vendors whose labels:
1. last a long time (around a decade);
2. resist chemical contamination; and
3. are reliably removable from the container when the occasion arises in the lab setting?
