The Division does have a Lab Incident Investigation form available at
https://urldefense.proofpoint.com/v2/url?u=https-3A__www.surveymonkey.com_r_chas-5Fll-5Fbeta&d=BQIFAg&c=lb62iw4YL4RFalcE2hQUQealT9-RXrryqt9KZX2qu2s&r=meWM1Buqv4IQ27AlK1OJRjcQl09S1Zta6YXKalY_Io0&m=UX8VyncPo3xQaEcUy1MO8mHq6ipIORfxiJ6Tihd_pEw&s=Fw6WlIh010xAr2hZR2tHaHKoj6uUzDdUvcZ2U-trci0&e=
It is a 2 page form; the first page is designed to describe the incident from the witness's point of view; the second is designed to help do a safety analysis of the event. We haven't pushed it out to the world very much because we're not clear how the information collected is best used. Which is why I raised yesterday's question.

The Division does have a peer reviewed journal, the Journal of Chemical Health and Safety. Information about it can be found at
https://urldefense.proofpoint.com/v2/url?u=http-3A__www.sciencedirect.com_science_journal_18715532&d=BQIFAg&c=lb62iw4YL4RFalcE2hQUQealT9-RXrryqt9KZX2qu2s&r=meWM1Buqv4IQ27AlK1OJRjcQl09S1Zta6YXKalY_Io0&m=UX8VyncPo3xQaEcUy1MO8mHq6ipIORfxiJ6Tihd_pEw&s=w_kXKhG1EaIMquBKw2l7hSy0KjuMHp0AnoJ-0qXPiwg&e=
Let me know if you have any questions about this.
