We use iFormBiorisk/iFormBuilder at Emory. I've also used it doing international reviews in low resource countries on projects for the Elizabeth R. Griffin Research Foundation.
I has been a great platform. Has pre-made templates, especially for lab safety and biosafety. In addition, 100% customizable and integrates into many databases.
Also the iFormBuilder staff an awesome to work with.
