Contact Kalpana Rengarajan here at Emory. We use our inspection program as an ongoing risk assessment/gap analysis. We now can show with data over the last several years as the program has evolved, reduction in deficiencies (gaps) and a safety culture
change with our faculty and staff. Last year we beta tested department score cards in 8 departments and will be going to score card in all departments this year. The program philosophy is now being implemented in our general facility safety and environmental
audits as well. If you will be at the CSHEMA conference this next week look either one of us up. Also, Kalpana and I will be teaching a 4 hour class on our program this Friday and Seth Salinas will be teaching a class on Saturday afternoon on the iPad platform
(iFormBuilder/iFormBiorisk) that we use for all our inspections. I used the iFormBioRisk platform this last month on a project I did for the Elizabeth R Griffin Research Foundation in remote areas of Kenya. We completed risk assessment to look for local
/ sustainable solutions. It worked flawlessly!
Kalpana and her group have been presenting the progression of the program at both CSHEMA and ABSA for several years now and have been published in the DCHAS journal. She can share with you the references as well.

Notre Dame is currently looking at conducting risk assessments(physical hazards as well as chemical,radiological and biological) of each of laboratories on campus. Has anyone done this before and willing to share the matrix you used?
Whether or not you have done a formal risk assessment, have you developed a process or identification factor(s) that ranks labs for an inspection schedule (i.e. Low risk - every two years, high risk =96every year, etc). If so, would you be willing to share?
