You know….this is a good idea, I think I am going to incorporate this into both my Lab Safety Management course as well as the lab safety training sessions I hold for lab assistants...for starters. Then I'll hit up the faculty in their training sessions. Little bit of brain storming here….and a good assessment tool too!

The one NYCOSH uses just has 4 questions. The first two ask you to grade the training on a scale of poor, satisfactory, good or excellent on how you liked it and how well it met your specific needs for safety information. Third asks for how you think the sessions could be improved, and fourth I think for any other comments.

I did a google search for such a survey, but most of the results concerned how 
training is delivered rather than the results from the participants' points of 
view. There is the approach of assessing safety climate, which could be 
considered the product of the training program, but I'm primarily interested in 
how the institutional safety training program itself is perceived at this point. 
It appears that I'll have to develop one myself.
