For example, for the cryogens and dry ice fact sheet (http://www.osha.gov/Publications/laboratory/OSHAquickfacts-lab-safety-cryogens-dryice.pdf) they show a person with no eye/face protection even though the fact sheet says: Always use a face shield in conjunction with safety glasses or goggles.
