Subject: Biological Material/ Tissue transfer form

Colleagues,

I am working on revising a Biological Materi al/ Tissue transfer form.  This is for materials that are coming from another institution into ours.  The primary basis is to evaluate if we have the infrastructure for the tissues, if the PI has the training to man age the tissues and to keep the IACUC and other pertinent committees inform ed of the research that is occurring.  As a note this is the research or education that is “not” managed by a grant or a foundation o peration.  2 questions:

1.       Who does this on your campus?

2.       Can you send me the template that you use?
