I knew a big pharma in Boston had a terrible one a few years ago and  
they banned pressure tube use in the lab. And, CENews (April 13, 2009)  
reported another one. I am wondering how many incidents involving  
pressure tube/sealed tube explosions per year? and the reporting  
requirements of such incidents?
