I wrote one for my labs in HyperCard back in the 90's.  HyperCard 
hasn't been supported for years and requires Mac OS 8 or 9 (or OS X 
running in Classic Mode), so that also means you can't use it on an 
Intel-based Mac or with Leopard.  And I just tried it with Tiger and 
hung it in Classic Mode, so you'd ideally need to use this on really 
old surplus Mac.

But if you want a copy, it was pretty slick.  Just about most things 
you might want except bar coding. And you can bulk import data from a 
spreadsheet.   It requires HyperCard 2.2. or higher, although it is 
available as a standalone application with the HyperCard engine built 
in.  You can customize lots of things (and everything if you own a 
copy of HyperCard).
