If anyone wants to write up their first-hand knowledge of accidents, I'm happy to revise the Safety section of our site and post them there: http://www.ilpi.com/safety/ I would ask, however, that such contributions go into very specific detail and post-accident analysis from the pedagogical perspective of "what went wrong and how could this have been prevented". See, for example, the write-up I already have posted, http://www.ilpi.com/safety/explosion.html Pictures are desirable, but not required. While I wouldn't have these in a database, the safety section can be made searchable with little effort. I have witnessed a number of similar (and more serious) incidents which I should get around to posting so that students can learn from them...I've mentioned some of them on the list before. And I will most definitely be adding one about a demonstration in which a middle school student lost an eye to a soda bottle that his teacher filled with dry ice and water (but that will have to wait until the court case is completed).
