Disclaimer: My company is a Guardian Equipment distributor.  Guardian 
makes eye/face washes and safety stations, nothing else.

The heads on Guardian's units (don't know about other companies) have 
in-line filters; these should be replaced as needed.  The interval 
really depends on your water hardness.  Once or twice a year seems 
prudent.  You can see a block diagram for Guardian's heads on page 2

You can always place a standard Bourdon-style pressure gauge on the 
supply line.  If that reads in the manufacturer's operating range and 
your filters are clean, then simple operation/function tests should 
generally obviate any need for flow testing.

I forwarded your inquiry Guardian and got their opinion on the flow issue:

"Guardian Equipment has all of our
products third party tested and certified to meet or exceed the ANSI
Z358.1-2004 standard.  The flow rate does not need to be measured by the end
user, unless for some reason they feel the flow is not meeting the minimum.
Our GS and FS Plus spray heads are designed with a flow control built in.
The GS head will deliver 1.8 GPM with a water pressure range of 20-100 psi.
The FS head will deliver 3.2 GPM with a water pressure range of 20-100 psi.
My suggestion would be for anyone to visit our website www.gesafety.com for
a copy of our ANSI Z358.1 checklist.  They can also send in a request for a
free eye wash testing template."
